At present Overseas Business Travel can only obtain extra pages for U.S. passports. To apply you need to provide the following documents:

• Complete the Application Form for Additional Visa Pages
• A letter from the applicant authorising us to act on applicant's behalf. Please refer to the sample letter for guidance. 
• Valid passport
• Completed and signed Record of Mail Order Credit/Debit Card Transaction.

You can only add extra pages into a U.S. passport a maximum of 3 times. Then the passport will need to be renewed.
